Adults Social Worker (Social and Healthcare Qualified)


This is a full-time position with the Mental Wellbeing service team in Southampton. This a specialist Mental Health and Substance use social care team that work predominantly within the Care Act, Mental Capacity Act and s117 of the Mental Health Act.


Job Purpose
To provide short-term support within the Mental Wellbeing Service, working alongside social workers, mental health practitioners, health colleagues, and partner agencies to deliver person-centred, strengths-based support to adults with mental health and substance use needs. The role will assist in promoting independence, managing risk, and ensuring individuals receive timely access to appropriate services and interventions.


Key Responsibilities


Support adults experiencing mental health and/or substance use difficulties to access services and community resources.
Work collaboratively with multidisciplinary professionals, carers, and partner organisations.
Maintain accurate and timely case records in line with organisational policies and procedures.
Assist with assessments, support planning, reviews, and safeguarding processes as directed.
Support individuals during periods of transition, hospital discharge, or increased mental health need.
Contribute to risk management planning and the promotion of safe, positive outcomes.
Attend team meetings, supervision, and training as required.
Contribute regularly to the duty rota.
Office based when on duty and at least once a week, more frequently when starting with the team.


Essential Requirements


Experience of working with adults with mental health/Substance use needs within a health, social care, or support setting.
Understanding of person-centred and strengths-based approaches.
Ability to work effectively within a multidisciplinary environment.
Good communication, record-keeping, and organisational skills.
Commitment to safeguarding, equality, diversity, and inclusion.
Knowledge of relevant legislation including the Care Act 2014 and Mental Health Act 1983.
Experience within Adult Social Care or Community Mental Health Services.
Car Driver.
